Dear Parents and Guardians,

As you wrap up the last few weeks of summer vacation with your kids, here is the required Medical Information form that is due upon return to school. Please remember that in North Carolina, certain immunizations are required for all students entering Kindergarten, 7th grade, and 12th grade, unless your student has a medical or religious exemption on file with the nurse. These immunizations must be completed on the first day of school.

This is the Medication Authorization form, which is optional for parents and guardians who would like their child to receive over-the-counter medications as needed while at school.  This form requires a signature from the student’s medical provider.
