Dear Parents and Guardians,

As you wrap up the last few weeks of summer vacation with your kids, here is the required Medical Information form that is due upon return to school.  Please have this form completed for the first day of school.

Medical Information and Emergency Release Form

This form is the Medication Authorization form that is optional for parents and guardians who would like to have their child receive over-the-counter medications as needed while at school.  This form requires a signature from the student’s medical provider.

If your child requires an EpiPen or Auvi-Q, please have your student’s medical provider fill out the following form.
